1. Company sponsored Health Insurance Plan:
a. BCBS Bronze (HSA eligible) and Silver (non-HSA eligible) Plan options for all employees (20 hours or more per week average) and their spouse and dependents.
b. Small Lot pays 50% of the employee (not spouse and dependents) monthly health insurance premium
c. Group insurance premiums are age adjusted by BCBS
2. Health Savings Account:
a. Employees purchasing Company-Sponsored BCBS Bronze plan are eligible for automatic payroll deductions to an individual HSA account for payment of eligible medical expenses.
b. Sponsoring organization is “Further”.
3. Company sponsored Dental Plan:
a. BCBS Freedom Enhanced Plan - $1,500 maximum annual benefit to Employee.
b. Small Lot pays 50% of the employee (not spouse or dependents) monthly dental insurance premium.
4. Company sponsored Vision Plan:
a. Blue Cross Blue Shield Vision Plan, called the “Value Standard with Exam’. The plan is summarized in the ‘Vision Plan and
b. The annual premium cost for employees (and spouse/dependents) is outlined in the BCBS Value Plan Annual
Premium brochure below.
c. Small Lot pays 50% of the employee (not spouse or dependents) monthly dental insurance premium.
d. Unlike the Medical and Dental plans, however, the portion of the annual premium to be paid by employees that accept the insurance plan will be paid through a single payroll deduction early in January of the Plan Year, or at inception of coverage (whichever comes first).
5. Direct Employee Beverage Discount Purchase Program
a. Employees are eligible to purchase Small Lot distributed beverages direct from Small Lot for personal consumption
b. Orders placed through Small Lot sales management software for “Will Call” purchase to be picked up at warehouse
c. Purchase price at the lowest price available to our retail and restaurant customers
d. Automatic deduction from payroll for the purchase of selected beverages
